Attached is the athletic form.  It needs to be printed, filled out and returned to the BCI PE teachers by May 31, 2012.  A physical is required and we recommend scheduling the appointment early.  Please make sure you and your child have reviewed the entire packet and all the blanks are filled in with the required signatures.
