Welcome Parents/Students to the 2016-2017 school year!

 ​Below you will find links/packets​ to some frequently used forms.

 Medication Authorization Form:  Required for student to receive any medication during school hours.

Over the counter medications- medication must be in the original container.   Over the counter medication can only be given seven times without a physician's signature. 

·         Any over the counter medications that contains Aspirin will require a physician's signature. For example:  Pepto-Bismol, Excedrin Migraine, etc.

·         A student will only be allowed to carry an Epi-Pen, Inhaler, or Diabetic Medication on his/her person with correct paperwork completed.  A student will NOT be allowed to carry any other medication on his/her person during school. 

·         All medication must be current, so please check expiration dates. Expired medications will not be administered.

·         Medications must be brought to the school and picked up by an adult.

 Asthma Care Packet:  Contains all forms required to care for your child at school.  Please have physician complete appropriate sections and return with medication.  Also if physician requires a student to carry an inhaler, please note that physician must have signed the form titled SELF ADMINISTRATION in order for them to carry inhaler on their person.

 Diabetes Care:  Your doctor will already have specific orders that he will give you for the school. In addition to this we will need medication authorization forms filled out for insulin and glucagon to be given.  These two forms will need to be filled out and signed by parent/guardian.  In addition to this,  a diabetic supply checklist for your child's diabetic needs is found below.  Please bring the supplies listed in a plastic box with lid, with your child's name on it.  A nurse will be getting in touch with you to schedule an individualized health plan meeting before the start of school.

 Immunizations:  Below you will find the link to the CDC Website in regards to the State of Texas Immunization Schedule for Texas Public Schools.  There is an English and Spanish version.  There is also a link on the website's page to the catch up schedule if you were not able to get your child's vaccinations in the timeline it should have been given.

Please examine your child's immunization record and keep track of when they are supposed to receive vaccinations.  This will help in enrolling your child and will prevent you from being denied enrollment or excluded in regards to this.  Please make sure you bring an updated Immunization Record when enrolling.

 Flu Season Note:  Below is a link to our annual flu season notice.  Please note if your child is experiencing any flu-like symptoms they most likely need to stay home to recover.  Any temperature at or above 100.0 will be sent home.  Please notify physician if your child is experiencing any symptoms to receive the appropriate medications to combat it.

 Overnight Field Trip Form:  Students that need to take any medications on overnight field trips this is the form that must be filled out.  Please note that any controlled substance must be stored with and given by a staff member.  Students are not allowed to carry any controlled substances at any time. Additional instructions are listed on this form.  Please fill out and return to your teacher in charge of the field trip by the given due date they so choose.​
